PDO Thread Lifting: What to Expect Before, During, and After

PDO threads sit in the awkward middle ground between Botox-and-fillers and surgery — non-surgical but more involved than an injection, longer-lasting than HA fillers but shorter-lasting than a face lift. Whether they're right for you depends entirely on what you're trying to fix and how much downtime you can tolerate.
What PDO threads actually are
PDO (polydioxanone) threads are absorbable sutures originally developed for cardiothoracic surgery. The aesthetic application uses thinner versions of the same material to physically lift sagging tissue while simultaneously triggering collagen production over the months following placement. The threads themselves dissolve in 6-8 months; the collagen they stimulate persists much longer.
Three main thread types we use depending on the goal:
- Smooth threads (Mono) — straight, no barbs. Used for general skin tightening and collagen induction. Bunny lines, marionette lines, neck.
- Barbed threads (Cog or Z-type) — small directional barbs that grip tissue and provide actual mechanical lift. Used for cheeks, jawline, brow lift.
- Screw threads — twisted configuration for volume in deeper tissue. Used in nasolabial folds and marionette areas.
Who's a good candidate
- Late 30s-50s clients with mild-to-moderate sagging — typically jawline, cheeks, brow.
- Clients who've considered a surgical face lift but aren't ready (financially, schedule-wise, or emotionally).
- Anyone with structurally good bone and skin quality but loss of definition.
- Lip flip enhancement, mouth corner lift (smaller applications).
Who's not
- Significant sagging — multiple inches of redundant skin. Threads can't hold against gravity at that scale; surgery is the right path.
- Active skin infections or inflammation in placement areas.
- Autoimmune conditions or known wound-healing issues.
- Anyone hoping for a one-and-done permanent fix.
The procedure
A PDO thread session at our White Plains location runs 30-60 minutes depending on areas treated. Topical numbing is applied; many providers (including ours) also do small injected lidocaine bumps along the thread paths for comfort. The threads enter through small needle-like cannulas (no real cuts), get positioned through the dermis, and are anchored at small attachment points.
You'll feel pulling and pressure during placement, not sharp pain. Mild bruising, swelling, and temporary skin dimpling at attachment points are normal for 2-5 days.
Recovery + results timeline
- Immediate: Subtle visible lift. The tissue is mechanically held in the new position.
- Days 2-5: Mild swelling and bruising peak. Avoid aggressive facial movement, sleep elevated, no heavy chewing.
- Week 2: Most external signs resolved. Lift looks natural.
- Months 1-3: Collagen production ramps. You actually look better at month 2 than at week 2.
- Months 6-8: The threads themselves dissolve. The collagen they generated keeps the area firmer than baseline.
- Months 12-18: Most clients return for refresh threads. Cumulative collagen quality improves with each round.
Costs in White Plains
PDO threads start at $300+ per session and scale up based on thread count and complexity. A typical mid-face lift uses 6-12 threads ($1,200-$2,500). A more comprehensive lower-face/jawline session can run $2,500-$4,000.
Combining with other treatments
- Sculptra for foundational collagen rebuilding (do Sculptra series first, then threads ~3 months later).
- Botox in the upper face to maintain a smooth complement to the lifted lower face.
- HA fillers (Voluma, Juvederm) for deeper volume restoration where threads alone don't fill.
